Please include a date and time that you prefer, as well as a phone number where we may reach you. You will promptly receive a response from our Front Desk Coordinator during our regular business hours to set up your appointment and gather insurance information for your visit.




Request Telemedicine

For our patients who are self-quarantined, or simply prefer to stay comfortably at home, we are now offering Telemedicine Services. You can Call or Text (770) 992-6789, to set up a secure video appointment with Dr. Zadeh.

Dr. Zadeh will be providing the same level of customized and compassionate care that he would in person. Please be advised though that certain eye conditions require testing and in-office treatment. If we cannot address your concerns virtually, we will make accommodations for you to see Dr. Zadeh in person. If you are uncertain of whether you require an in-person visit or not, contact us and we will assess your needs and symptoms.

Previous Patient: Upon check-in, we will verify your current information on file.

Record Release Form: If you need to request records for yourself or another doctor's office, please print and fill out the "Record Release Form" and fax to: (770) 640-6789 - Attn: Front Desk. Releasing records can take up to 2 business days for active patients and may take longer to retrieve old records. Thank you.
